A list of all dentists in practice in Scotland and their address and postcode enclosed in an excel spreadsheet.

Provide a list of dental practices in Scotland disclosing for the most recently available year their:

1. General Dental Practice Allowance
2. Remote Area Allowance
3. Rental Allowance
4. Rates Allowance
5. Principal Dentist
6. Practice Address, telephone number, email address
7. If that can be provided via a spreadsheet, with each column representing a different query, that would be very helpful. 

In addition, I should be grateful if you would provide a list of all dental body corporates registered in Scotland, together with the practices that are registered to them.

I understand GP's running their own practice (ie: not gp's employed directly by nhs), are paid a retirement pension by the NHS. If so, what is the annual value of such pensions?

I am requesting a list of all pharmacies in Scotland in a spreadsheet format.

Please provide NHS Dental Practice Income, Allowance Claims, Registration & Participation, Dentist Count, for previous 5 years. 

To be arranged into Health Board Areas, for the last 5 calendar years, Jan – Dec 2013-2017. 

Information can be arranged on separate tabs similar in layout to FOI-2017-000229, but always arranged into separate Health Board Areas.

I would like to request information relating to payments made for prescriptions for unlicensed medicines using the process described in NHS Circular PCA(P)(2015)17 whereby the prescription is endorsed (either on paper or electronically) with "hc 3000" to claim for the £30 Standard Handling Charge. 

For each Pharmacy Contractor (giving their contractor number, pharmacy name, address and post code) for the last 12 months, broken down monthly:

• Month, Item description, total quantity dispensed in that month, number of £30 handling charge claims made. For example:

"July-17, Diltiazem 2% Cream (30g), 2 packs, 2 claims”.

Please provide a list of all NHS Dental Practices in Scotland as well as -

• Individual name of the rate payer for their respective practice.

• Sums paid to each individual NHS dental practices for the last and current financial year in respect of payments for the reimbursement of practice rental costs and reimbursement of non domestic rates.

• Latest figures of the number of dentists with a list number within each individual practice.

Information requested -

• Number of patients registered in each dental practice for the year 2014, 2015, 2016.
• Dentist earning in whole of Scotland for the year 2014-15, 2015-16 and 2016-17.
• Payments made to all dental surgeries for the years 2014-15, 2015-16 and 2016-17.

Please provide a list of all NHS Dental Practices in Scotland along with their:

(i) gross NHS income from March 2016 to February 2017

(ii) latest NHS patient registration figures

(iii) Latest figures of the number of dentists with a list number within each individual practice, and

(iv) Sums paid to each individual NHS dental practices for the last financial year in respect of payments for the reimbursement of practice rental costs and reimbursement of non domestic rates.

1. Names and amount paid to top 200 dentists in NHS Tayside area for the last financial year ending April 2017.

2. Number of patients registered for the above dentists at the end of March 2017.

3. Amount paid to top 50 dental practices in NHS Tayside area for the last financial year ending April 2017.

4. Names and amount of grant paid for SDAI to every dentist in NHS Tayside area in the past 8 years.

5. Name and amount paid to every NHS dentist in Scotland in the past financial year (from the highest to the lowest).
