GP Registration - Data Sharing
To register with a new GP practice, section 5 of the Patient declaration of the NHS Scotland application form states: "To enable NHS National Services Scotland to confirm eligibility to lawfully register with a GP and for the purpose of prevention detection and investigation of crime, relevant information from this form will be disclosed to the NHS Business Services Authority, NHS National Services Scotland, The Home Office, Identity and Passport Service, HM Revenue and Customs, the General Register Officer and Local Authorities".
- Could you please advise how this sharing takes place?
- Why all of the above would be interested in a new GP registration?
- Why there is no paperwork that needs to be signed for a temporary registration or indeed any requirement to advise a temporary patient on how their data is shared?

Number of prescribed items dispensed
- The name, ODS code and postcode of each pharmacy.
- Total number of prescribed items dispensed by each pharmacy from 2012/2013 to 2015/2016.
- List of enhanced services provided by each pharmacy in 2015/2016.

Items dispensed and NHS payments
For pharmacy contractor 6106:
- The total number of items dispensed per calendar month, for the last three financial years.
- The subsequent NHS payments made to the contractor for each calendar month, for the last three financial years.
- A breakdown for AMS, EMAS, PHS and CMS payments for each calendar month, for the last three years.
