Accessibility statement for Formic surveys and forms
This accessibility statement applies to the following surveys created using Formic’s Fusion Online Forms.

These surveys are run by Education Scotland.

How accessible is this website?
We know some parts of the surveys are not fully accessible:
- checkboxes are announced as buttons when using a screen reader.
- colour contrast is insufficient
- text input fields do not have associated labels
- links opening in new tabs are not announced to the user
- some parts of the forms are difficult to navigate using just a keyboard
- Content does not reflow when browser zoom is applied
- Section which is not relevant to the user is focusable and announced by screen reader
- Dropdown menus inoperable using keyboard alone
- Use of sensory characteristics when instructing users how to interact with a call to action.

What we’re doing to improve accessibility
Formic Solutions are working to address the areas of non-compliance detailed above and aim to deliver a new version of their Online Forms product that conforms fully with WCAG 2.1 by the end of Q4 2020.

Preparation of this accessibility statement
This statement was prepared on 3 September 2020. It was last reviewed on 3 September 2020.
This website was last tested in March 2020. The test was carried out by User Vision.
The following approach was taken for the accessibility audit by User Vision.
- Automated evaluation using Accessibility Insights, Powermapper and Wave.
- Manual accessibility evaluation to code level using browser code inspectors, an approach that identifies issues that cannot be captured by automated tools.
- We viewed the application in a range of browsers:
- Chrome, Firefox and IE on desktop.
- Safari on iPad & iPhone.
- We assessed pages using the following assistive technology:
- NVDA (with Firefox and Chrome) screen reader for desktop.
- VoiceOver for iPad & iPhone (iOS).
- Talkback for Android
- We checked colour combinations using a colour contrast analyser to determine if there is enough contrast in the visual presentation.
