Patient letters

Question

1. A copy of the template of the initial letter sent to patients querying their entitlement (ie: prior to a Penalty Charge Notice (PCN)).

2. A copy of any templates for follow up letters covering:
i) patients who successfully evidence their entitlement;
ii) are unable to evidence their entitlement, and are presented with a PCN;
iii) fail to reply and are issued with a PCN; and
iv) any other template.
