Get a paper PVG form or guidance documents

The quickest way organisations can apply for all disclosures is by email.

If you cannot complete a PVG application by email, you can ask Disclosure Scotland for a paper application form. This takes longer, so try to apply by email first.

You must use a paper form:

  • if you are applying to register your organisation with Disclosure Scotland
  • to add or remove a countersignatory.

Email response@disclosurescotland.gov.scot to get paper forms. Include your :

  • name
  • address details
  • the number and type of application or form you require
