Freedom of Information disclosure log

I would like the following information released under the Freedom on Information Act.           

1. Any meetings, correspondences, notes, reasonings and logistics for the fee-scale given in the document entitled 'Discontinued Fees' as applies to Orthodontic treatment?

I am interested to know the basis on which this fee-scale was determined, as I feel that the fee scale is disproportionate to the work undertaken. 

2. Any meetings, correspondences, reasonings, notes, reasonings and logistics for changing the existing system for the leaving orthodontist to be paid for their share for treatment undertaken by them, only after the 2nd orthodontist has completed that patient's orthodontic treatment?

I am interested to know what has been discussed to mitigate the patients' treatment being adversely affected because of this decision, as it will encourage orthodontists coming into UK for short period of time and then leaving for financial gains.  The existing system safeguards against this and ensured continuity of treatment for the patients in Scotland.

I would like to request a list of GDP’s in practice in Scotland and for this information to be provided in an excel spreadsheet:

Full name, practice address and postcode

Please could you forward me the latest figures for:

1) 100 top paid NHS dentists in Scotland (breaking down into capitation, continuing care and item of service),

2) Top 30 dental practices that have received the General Dental Practice Allowance

3) Top 30 highest Seniority Payments made and to whom.

1.  How many dentists were suspended from the NHS performer list in Scotland in each of the past five years?
2.  How many dentists had conditions placed on their record on the NHS performers list in each of the past five years?

1.  The value of payments recovered each year from dentists in Scotland by Practitioner Services between 2010 and 2020 (for example, the annual report of the Scottish Dental Practitioners' Board (SDPB) for 2010/11 notes: "Throughout the year, PSD - working to the policies of the SDPB - has … recovered circa £900,000 from practitioners.

2.  What baseline recoveries value was agreed by Practitioner Services and the Scottish Government for each of those years (for example, in May 2018, the minutes of the SDPB noted: "A baseline recoveries value of £200,000 had been agreed with the Scottish Government.")?”

1) Currently, how many beneficiaries are registered with the Scottish Infected Blood Support Scheme?

2) How many of the beneficiaries are in each category? I.e. Self categorised not affected, moderate, severe, HCV stage 2, HIV, co-infected, widows.

3) Since July 2017, the announcement of the Infected Blood Inquiry, how many Scottish victims have died? & from within which category?

4) What is the current known amount of Haemophiliacs beneficiaries registered with SIBSS?

I was wondering if I could get a FOI for the value paid out from the NHS to every single Opticians in Scotland. 

Further, if this could be broken down into: 

1. Full name of practice, including: address, phone number and email address
2. Any part one registered optometrist
3. Eye tests broken down into type. ie over 60, under 60 or supplementary etc.
4. Any vouchers that were paid
5. If any patients with cataracts were present.

A list of all NHS GP surgeries in Scotland along with the nhs.net email address held for the surgery and the name of the practice manager for each surgery if held.

Please supply the following information: 

Total Number of Optometrist in Scotland registered on the NHS ophthalmic list. 

Total number of IP optometrist in Scotland. 

Financial year 2018/2019.
