• Could you please tell me how many patients the company Optical Express has treated on behalf of the NHS?
• I'd also like to know which procedures they have undergone and over what timescale.
• I'd also like to ask if this information could be broken down into areas of Scotland, ie X patients in NHS Highland area and so on.
• Could I also ask how much the company has been paid by the NHS for this work?

In each of the past five financial years, 

1. The number of fraud allegations that have been made in Scottish health boards. 

2. The number of fraud investigations that have been carried out, broken down by the relevant health board if possible. 

3. The number of investigations that have made a finding of fraudulent activity, broken down by the relevant health board if possible.

My questions are in relation to registration and use of chronic prescribing:

1. What was the total number of patients registered for CMS end 2017 2018 and now?
2. I know you analyse the number of items actually prescribed under CMS monthly can you provide a total for 2017 2018. And first quarter 2019
3. Is it possible to link the number of registrations and prescriptions to each GP practice?
4. Can you provide an analysis of number of registered patients in each Board area and the board population as demonisation in each case?
5. What was the total cost of registration in 2017 and 2018?
6. Has any evaluation of the chronic medicines prescribing system been undertaken and if so published or made available online?
7. The GPs and other prescribers still have to sign manually all scripts. Are there any plans to move to electronic signatures? If not what are the barriers to this?
8. As I understand it the scripts are still passed manually to the community pharmacist who then have to enter the details onto computer for dispensing and pricing. Is this now digitised? Are there any plans to allow electronic (secure) transmission electronically from GP and others to the community pharmacist to eliminate the intermediate step?
9. Do community pharmacists have computer access to the emergency care record? If so is this for all patients or just for CMS registered patients?
10. Is the fact of CMS registration along with details of the community pharmacy noted on the emergency care record?

For the last 3 financial years of 2016/17, 2017/18 and 2018/19 please provide details of all payments made under Determination XI of the Statement of Dental Remuneration, for Clinical Audit / Quality Improvement in Scotland.  Information to be grouped by Health Board area, and payments within each Health Board Area to then be arranged by 1/. Name of dental practice 2/. Name of contractor recipient  3/. amount claimed.

My request is for the information pertaining to the gross earnings of dentists who have received maternity payments (specifically the gross earnings for the test period, 21 months prior to confinement) and for their gross earnings for the 12 months prior to confinement/commencement of maternity pay. I would like this information about dentists who received one or more maternity payments between the period 31st May 2014 to 31st May 2019.

Thus for each dentist who has received maternity payment I would like:

• Gross earnings for their maternity test period as per the SDR (21 months prior to confinement).
• Gross earnings for the 12 months immediately prior to confinement/commencement of maternity payments.

My request is for the information pertaining to the gross earnings of all dentists who have received maternity payments (specifically the gross earnings for the test period, 21months prior to confinement) and for their gross earnings for the 12 months prior to confinement/commencement of maternity pay. I would like this information about dentists who received one or more maternity payments between the period 10th April 2014 to 10th April 2019. 

Thus for each dentist who has received maternity payment I would like: 

• Surname,
• Date of birth,
• Gross earnings for their maternity test period as per the SDR (21months prior to confinement),
• Gross earnings for the 12 months immediately prior to confinement/commencement of maternity payments. 

As this requested information spans over multiple financial years it would be useful if this information was delivered in tables or Excel spreadsheets specific to each financial year.

Please note this is not a request for information for paternity or adoptive leave payments. I only wish to receive information about maternity payments

Through FOISA, I have it confirmed that 21 Scottish Victims registered with SIBSS have passed away between July 2017 and 31 March 2019. My FOI request is as follows:           

• How many of the 21 were Haemophiliacs?

Please provide the details of payments made to NHS dentists for 2018-19 and the number of patients registered within NHS practices across Scotland.

I am looking to understand NHS Scotland's overall spending on special contact lenses (i.e. used to treat any irregular cornea conditions).

How much has the NHS Scotland spend annually on contact lenses by product name (i.e. Kerasoft IC), supplier name (i.e. UltraVision)  and contact lens types (i.e. RGP, Soft etc) in the last 2 years covering 2016-2018?

When was the policy which enables patients to amend the sex category under the Community Health Index introduced? 

Please provide full details of the consultation undertaken prior to the introduction of this policy, in particular:

What risk assessments were undertaken?

Which of the Royal Colleges and other professional bodies were consulted?

What consultation was there with the Chief Medical Officer's department?
