Please could you supply the following listed information relating to General Dental Services in Scotland.

Please provide the current activity of dental practices measured by PSD in percentage terms (or other measure used by PSD) since the reintroduction of the SDR in November 2020. If possible provide activity details from the previous year for comparative purposes.

Additionally, please provide this information in an excel spreadsheet, by healthboard, by individual practices and by month.

Would you be able to provide the list of dental practices: Dental practice names, Full address and postcode and full names of dentists operating at these practices please? Including names of all dentists operating at each practice?

I am seeking information regarding free prescriptions in Scotland. Can you please provide figures for each of the last four years (2017-2020) regarding the number of free prescriptions which were given out in Scotland? Can you also provide numbers for these years which outline what % of prescriptions were given for certain medical issues such as psychological medicine, pain killers etc. In addition, can you give figures for the number of prescriptions which were approved by a GP but never picked up from the chemist for said years.

I wondered if it was possible to find out the total number of Optometrists registered on the Ophthalmic List, and the number of Scottish Optometry practices in 2020.

This data had been requested previously for the financial year 2018/19 however a more current figure would be beneficial.

The schedule of payment details to dispensing practices from NSS Practitioner Services and also the current clawback rate and dispensing fee for dispensing doctors?

Please provide a list of each community pharmacy in Scotland detailing: Pharmacy name, address with postcode, related NHS board, the total number of dispensed NHS items in the previous calendar year and the total amount paid to each contractor in a previous calendar year.

I read an article in The Guardian stating that esketamine has been approved as a nasal spray for severe depression in the UK. I have suffered from severe depression for 20 years. Can I be considered for this treatment through my GP and if so, do I have to pay for the prescription? Any feedback and advice on this would be greatly appreciated.

I am looking to find out if anyone in Scotland has been able to get full set of dental implant's with help from NHS on medical ground and what medical ground they would have been. i.e. epilepsy. 

I am looking for the proposed contract document that a newly qualified independent optometrist who wanted to offer NHS GOS to his/her patients in Scotland. As you deal with Business Support I assumed that your department would have this information so that no fraud is committed. If your department does not have this application form who do, I contact?  I would appreciate the second request by return of email if that is possible.

If it is an application form that shows what criteria a new optometrist has to meet would be helpful.
