I am looking to find out if anyone in Scotland has been able to get full set of dental implant's with help from NHS on medical ground and what medical ground they would have been. i.e. epilepsy. 

I am looking for the proposed contract document that a newly qualified independent optometrist who wanted to offer NHS GOS to his/her patients in Scotland. As you deal with Business Support I assumed that your department would have this information so that no fraud is committed. If your department does not have this application form who do, I contact?  I would appreciate the second request by return of email if that is possible.

If it is an application form that shows what criteria a new optometrist has to meet would be helpful.

How many Patients have been deregistered by dental practices each month since February 2020 broken down by NHS board area.

I am looking for the trends in adult population numbers over the age of 65. Both for UK and Scotland.

Please provide me with a copy of the procedure and process to be followed by GP Practices when a patient request to deregister from that Practice. To be clear, I am referring to deregistration only with no transfer to any other Practice or provider.

Number of patients registered at NHS Scotland dental practices by practice name and location (address).

Could you please send me a list of the registered optician outlets in Scotland, their contact details (address including postcode, telephone number and email if possible) and the amount they have claimed against each type of optical voucher in 2019?

If the voucher request is too complex, could you please just send me the list of opticians and their contact details?

Would it be possible to have a PDF version of the 2010 Statement of Dental Remuneration?
