The Person-centred Health and Care Programme aims to support improvements in Person-centred care and practice across Health and Social care organisations in Scotland. Applying person-centred practices within health and social care services allows people who use those services to develop the knowledge, skills and confidence they need to more effectively make informed decisions and be actively involved in their own health and care. If people who work in public services are being person-centred they will ensure that care is personalised, co-ordinated and enabling so that people can make informed choices, manage their own health and live independent lives, where possible.
The way people experience health and care is an important component in delivering quality support and care services, as evidence shows that a focus on person-centred care can result in improvements in health behaviours, and health and wellbeing outcomes. Delivering this for people who access health and care services also requires the development of person-centred cultures for the workforce that support their wellbeing and provide more fulfilling work environments.

- Building capabilities in Person-centred Practice
- Developing and supporting a Person-centred Care Network of Practice
