Please provide the details of payments made to NHS dentists for 2018-19 and the number of patients registered within NHS practices across Scotland.

I am looking to understand NHS Scotland's overall spending on special contact lenses (i.e. used to treat any irregular cornea conditions).

How much has the NHS Scotland spend annually on contact lenses by product name (i.e. Kerasoft IC), supplier name (i.e. UltraVision)  and contact lens types (i.e. RGP, Soft etc) in the last 2 years covering 2016-2018?

When was the policy which enables patients to amend the sex category under the Community Health Index introduced? 

Please provide full details of the consultation undertaken prior to the introduction of this policy, in particular:

What risk assessments were undertaken?

Which of the Royal Colleges and other professional bodies were consulted?

What consultation was there with the Chief Medical Officer's department?

• Could you please tell me how many patients the company Optical Express has treated on behalf of the NHS?
• I'd also like to know which procedures they have undergone and over what timescale.
• I'd also like to ask if this information could be broken down into areas of Scotland, ie X patients in NHS Highland area and so on.
• Could I also ask how much the company has been paid by the NHS for this work?

Please provide contact general email addresses for all surgeries in all health boards.

1. A copy of the template of the initial letter sent to patients querying their entitlement (ie: prior to a Penalty Charge Notice (PCN)).

2. A copy of any templates for follow up letters covering:
i) patients who successfully evidence their entitlement;

Could you please break the equipment down by make, model, type, value and quantity rather than just as a total for optometry/ lab equipment?

2. The total number of patients who receive a letter in the initial 28 day period querying their entitlement, by year from 2010-present. 

3. The total number of patients who respond to the letter documenting their exemption, by year from 2010-present. 

4. The total number of payments made by patients before a PCN was issued, by year from 2010-present. 

5. The cash value of payments made by patients before a PCN was issued, by year from 2010-present. 

6. The total number of patients who fail to respond to the letter, by year 2010-present. 

7. The total number of the above exemptions sought that were subsequently issued with Penalty Charge Notices (PCNs) by year from 2010-present. 
8. The total number of Penalty Charge Notices that were paid, by year from 2010-present. 
9. Total revenue generated by Penalty Charge Notices, by year from 2010-present.  
10. Any breakdown of the number of PCNs issued for exemptions rejected according to evidence surrounding each of the following by year from 2010-present:

• B. Income-based Jobseeker's Allowance

11. Of the figures identified in 7 (above) how many of the penalty charges issues were appealed against, by year, 2010-Present? 

12. How many of these appeals identified in 11 (above) resulted in overturning of the charge 2010-present? 

13. The number of patients receiving a surcharge by year 2010-present. 

14. The number of patients actually paying surcharge by year 2010-present. 

15. The revenue raised via the surcharge by year 2010-present.
